I'm agree with both of you. We had experience the same failure in the past in Venezuela.

Our Caterpillar Agent recommends the same troubleshooting procedure.

These types of problems are normally associated with poor/improper external piping support or problems with the flexible connections. Piping movement and vibration gets transmitted to the housing, and the usual result is coolant weeps and then bolts loosening as you describe.

 

Is your piping mounted, supported and isolated in accordance with the CAT Application and Installation Guide?

 

Some pictures and additional information might get you some better answers, like serial number, rating, application, etc.

Mounting bolts Breaking for Cat 3616 Coolant Distribution block

We have had 2 instances where all the bolts either vibrated out or broke.  The last time we replaced the block.  I checked the bolts today and found several loose bolts and a few broke bolts on our units.  Anyone seen this problem and been able to identify the cause?
